Yeah there was actually a controversy around it, people didn't let it fly. She even had to come up with an alternate cover.

The people that were mad have to be some of the dumbest and most sensitive people. They completely removed all context and just saw a picture to have a knee jerk reaction to. Literally zero consideration of her expression, the album title, its content, her personality/views, or artistic interpretation. A woman on her knees having her hair pulled? She's clearly being abused and/or a total slut!!
