ā“ EXTRA FAQ BASED ON THE SUB’S DISCUSSIONS/ FOR THOSE NEW TO MH TOPICS

⚠ Disclaimer: This is not to be taken as a substitute for professional advice and just for sharing/ informational purposes.

šŸ”· Should I go for Government or Private?

Government services typically offer more affordable and subsidized healthcare especially when it comes to obtaining prescribed medication, however may have longer wait times and limited appointment flexibility. Some members have also shared that you may see a different doctor every visit, as typically when going to a government hospital or clinic.

While private services are usually more expensive, some may offer more options with faster access, options for more specialized care, have more flexible options such as online sessions or sessions outside of typical office hours, and may be easier to get in touch with their management compared to government services.

šŸ”· How do I seek help if I am a minor (below 18 years of age)?

For those who are under 18, legally you will require the consent/permission of your parent or guardian to seek healthcare or mental healthcare from clinics and hospitals (see:Ā Reddit post).

Other options available for minors, to talk to someone:

  • Getting in touch with your trusted school counsellor for counselling sessions
  • Buddybear ChildlineĀ - 1800-18-2327 (Toll- Free) (check link for available times) or FB Messenger

šŸ”· When do I know if I should seek help from a mental healthcare provider?

AĀ good question to ask yourself isĀ how much are your mental struggles interfering with your quality of life.Ā What some things could be possible mental health-related symptoms (list not according to a specific condition):

  • Has yourĀ low mood/ low motivation/ feeling mentally exhaustedĀ felt like it has been lasting for too long?
  • Have you been feeling painfullyĀ nervous/ anxious/ worriedĀ and it is affecting your daily life?
  • Has yourĀ anger been so hard to manageĀ it is damaging your relationship with loved ones?
  • Are you going through a difficult life situation and you find the emotions too overwhelming to cope with (eg. grieving a loss, relationship challenges, career/academic stress, burnout, etc.)
  • Are things that have happened in the pastĀ still negatively affecting you or cause overwhelming emotions, even though it has been long past the incidences?
  • Are you known to beĀ chronically late, forgetful or unable to plan ahead?
  • Are you struggling with anĀ addiction?
  • Is doingĀ certain activities (e.g. going outside, speaking in public, meeting large groups of people) really distressingĀ in general comparison to others?
  • Do you feel you are experiencing something abnormal, likeĀ hearing noises/ voicesĀ even if they aren’t there, orĀ feeling like being watched?
  • Any otherĀ recurring behavioral pattern or mental struggleĀ that often negatively affects you.

Should you feel unsure about your mental health and wish to speak to a professional to get recommendations on mental health assessments, tools or therapy;Ā don't let anyone's negative judgement/stigma on visiting a mental health professional stop you from reaching out.Ā Choosing to reach out and access mental healthcare with confidentiality are well within your rights.

At the end of the day, whether you have a condition or not you deserve help and support for the mental struggles you are facing.

šŸ”· What’s the difference between licensed counsellor/ clinical psychologist/ psychiatrist? Who do I go to?

šŸ”øCounsellor * Counsellors provide talk therapy and can help with situational or shorter-term challenges, such as stress, academic, career or relationship issues. * They focus on therapeutic support, coping strategies, and personal growth. * They do not provide clinical diagnoses or prescribe medication but may refer clients to other professionals for more complex or long-term concerns.

šŸ”øClinical Psychologist * Clinical Psychologists specialize in diagnosing and treating mental health disorders and are qualified to diagnose personality disorders based on criteria from diagnostic manuals such as the DSM-5, for eg. for conditions like ADHD, clinical depression, OCD, etc. * They may use psychological assessments, standardized tools, and clinical interviews to identify conditions, including personality disorders. * Some clinical psychologists focus primarily on assessments, while others also provide talk therapy. * They do not prescribe medication.

šŸ”øPsychiatrist * Psychiatrists are medical doctors specializing in mental health. They can prescribe medication. * They can diagnose, prescribe medication, and provide treatment for mental disorders. * Some members have shared that some psychiatrists may tend to focus more on medication and less on talk therapy during their sessions.

šŸ”· I have concerns regarding confidentiality when getting help for mental health.

In general, mental health practitioners must adhere to strictly practicing confidentiality, although they may be required to inform a third party if the patient is found to be at great risk at harming themselves or someone. Try checking the comments ofĀ this post question - Confidentiality on drugsĀ if you would like a better explanation.

šŸ”· How do I go about my first visit and what can I expect from therapy?

SeeĀ r/malaysia’s mental health wiki -Ā What to Expect in Therapy

šŸ”· What about online assessments/quizzes I found online? What do I do if I'm concerned with the results

It’s common to find mental health-related quizzes and assessments online, such as the DASS-21 test, which is often used for pre-assessments and can help you gauge your current emotional state (e.g., anxiety, depression, or stress levels). However, āš ļø these tools are not clinical diagnostic tools and cannot replace a professional diagnosis.

  • If the results of an online test concern you, consider sharing them with a trained mental health professional. They have the expertise to interpret such results in context and can provide further insights, guidance, or treatment recommendations.

  • It’s also important to note that mental health symptoms often overlap or may be caused by other conditions. For example:

Depression-like symptoms might result from physical health issues such as hypothyroidism. Mental health conditions may coexist (e.g., anxiety as part of ADHD or depression). Because of this, a clinical diagnosis should only be made by a qualified and certified professional, such as a clinical psychologist or psychiatrist. Websites or unlicensed individuals cannot legally or reliably diagnose mental health conditions.

šŸ”·Why do some cases require medication?

Some mental health conditions affect a person's nervous system, and medication may be needed to support it to function properly. Similar to how a diabetic patient requires insulin to support their health while a normal healthy person doesn't, a person with ADHD may be prescribed medication such as Ritalin to help them with their daily function.Ā Taking prescribed mental health medication is not a crime when it means supporting a person's health and wellbeing.

Mental health medication requires monitoring from a qualified psychiatrist, as every individual's mental health is unique. The treatment may involve trial-and-error to find the fitting dosage and type, and what works for one person may not work for another. In addition, if the medication causes unpleasant side-effects or is not effective, the psychiatrist may recommend a change in dosage or type. Mental health patients should work with their psychiatrist and avoid changing their medication plans without consulting them.

Many have shared that if prescribed, medication is just one part of supporting one's mental health condition, and it is still important to get talk therapy to learn how best to navigate one's mental health condition.

šŸ”· What can I do while waiting to get help?

Self-care,Ā as in ensuring your basic needs such as daily nutrition, some exercise, hydration, hygiene, and your emotional needs are taken care of. It is understandable if you are under for e.g. a depressive episode, these may be easier said than done, so do what you can do at the moment.

Also note that everyone’s form of self-care and preferences may be different. Some people may find solace by spending time with their interests, in nature, or engage in physical activities like taking a walk. Some may find it helpful to do journaling or meditation. You may also try breathing exercises or grounding techniques to calm down for the moment. If one method doesn’t work for you, never force yourself - pause and you always can try something else later.

While waiting, you can also see if you canĀ find support groups or informative resourcesĀ such as books / online resourcesĀ that may give insights and tips for what you are facing. Being part of a well-moderated group where you can listen or share your thoughts with others of similar struggles can be a great complement to your mental health journey.

šŸ”· I have been going to therapy for quite some time but my therapist seems to go nowhere/ frequently invalidates me/ did some things that seems unprofessional… but I am feeling unsure. Should I switch?

There can be 2 sides to this. From the therapist side, it may be the client may have been uncooperative such as frequently missing appointment, not following up on homework, not telling the truth, etc. In addition to consider is that therapy can take time, as the first few weeks, known as intake period are usually for information gathering. After that, the work may also take time to yield results while the client communicates with the therapist what works and what isn't working.

However, if for enough time you know you have been doing your due diligence as a client but the above question frequently occurs,Ā it is totally alright to consider switching to another therapist. The reality isĀ it is actually not uncommon for people sharing that they had to experience taking several tries before finding the right fit. Sometimes, it might be the therapist and client might just not be a good match. Or it might simply be that you were matched with one not able to assist you in your area of need or an unprofessional one. But just like dealing with any other service, you deserve to at least be treated with basic professional care from whichever therapy route you chose. Furthermore, if a therapist behaves unethically or violates boundaries, clients should report them to the relevant licensing board or seek help from a trusted authority if able to.

Do also consider looking for a specialist in your area of help required.Ā Examples being: If you have trauma symptoms, try to look for a trauma-informed therapist. If you feel your diagnosis results may have been overlooked and the therapist's explanation was not satisfactory, it is not wrong to seek another opinion from another specialist. If you are more aware of your needs, you can raise questions with your future therapist before engaging them.

We all know getting help isn't always a straight-forward journey, but that's why subs like this exist. Reach out to the sub if you have any other questions.

How do I get treated when I don't even know what's wrong with me?

• Upvotes

I'm a minor, well obviously by my pfp but I have a question. I know I'm mentally ill, I've had suicidal thoughts and have acted upon them (mostly threatening myself with a knife to my mom) and I have a very harsh background (abused ever since I was a few months old) but my mom refuses to acknowledge that I have mental problems.

Based on my own research, I think I have AuDHD, depression and bpd (I don't know yet, but I want to get it checked out). Most of my mental problems comes from school, family dynamic, years of being lied to by my mom and my mom herself. I suspect I have way more mental illness than the ones I just listed above, how do I convince my mom to take me to the hospital for a check up? She's a undiagnosed narcissist too so idk

If anything, I just want to get diagnosed either way. Either with my mom or without my mom. Actually, can I, as a minor, get diagnosis/treatment? My teachers asked me why I always ponteng once a day in a week and if my mom cared or not and ect..I think they know my situation or smth idk

Seeking Advice: KKM Counselling Return Call?

Hi there, long time lurker but this is my first post here. I have looking for mental health care for a long time after various stressors and I believe my depression has spiraled to an unsustainable state. A few weeks ago, I reached a point of crisis where I was crying nearly every day for weeks, and decided to take the advice of heading to the nearest KKM to get a referral.

I followed the standard procedure of getting a GP appointment via MySejahtera, but upon reaching and seeing a doctor, I was told that I would be referred to counselling and that the counsellor(?) or anyone relevant will call back to make arrangements. I asked for a rough timeline, or who to expect but the staff nurse was unable to give me any concrete details. I understand that waiting is inevitable, but wanted to know if anybody experienced this as well, and what steps I should take next.

I am worried that my mood will worsen again as my suicidal ideation flares up at times, and I am afraid I will gain the courage to take drastic measures otherwise. Private is not preferable for me at this time as I have recently become unemployed. I intended to get a referral straight to Psychiatry with my initial visit, but was also told counselling was the first step.

Appreciate the help.

Edit: I forgot to mention that my priority is to get on medication first and foremost, which is why I am stuck wondering about the KKM wait times. Initially I assumed I would be given the psychiatric referral on the first GP visit instead of counselling, thus I am wondering when they will be getting back to me.

Psychiatric Ward Process and Experience?

Hello.

To make a long story short, I've been on the verge of losing it. Things like memory loss, really bad moods, all that. A part of me doesn't like that I've been thinking of self-deletion so much more than usual.

I'm supposed to be taking Luvox but I don't have the courage to show up after skipping an appointment due to last-minute changes.

So I'm curious if anyone here has been in a psychiatric ward (and which hospital, if possible) and what the process was like. I'm desperate in being admitted for the safety/peace for everyone else around me.

Any additional details (like the conditions of said psychiatric ward) would be appreciated.

Many thanks. ā˜ŗļø

I apologise if this was badly worded.

Seeking Suggestions for Adult ADHD Coaching (Specifically Time Management) in Klang Valley

No formal diagnosis but I do feel I have mild ADHD. I really struggle with time management and self management. Mostly can manage but becoming a new father, it has become really tough.

Specifically looking for a coach (rather than a counselor/therapist) as my company provides an allocation each year to claim for coaching. Usually it's personal coaching but I am hoping to use it for this.

Thank you and really appreciate any recommendations.

Also if possible, I would also want it where I can opt out medications for my treatment but rather just behavioral or cognitive therapies. Thanks :>

Transferring case between hospitals

Hi fellow Malaysians. I wanted to ask if anybody here has experience transferring their case/records from PPUM psychiatry to Hospital Shah Alam. I would like to know the how would I go about requesting it?

I used to go to PPUM for ADHD, stopped few years back because of severe time constraints from work. Recently there have been new issues where I think I should go back, but currently I live in Shah Alam so going to PPUM is no longer convenient (besides, my super awesome doctor has left ppum)

Help

I, 20F, has been struggling really hard and this is my last ditch effort, Im barely scrapping by w money so I cant really get top notch or any notch professional care. I’ve been really struggling hard w memory loss for the majority of my life where i dont remember my childhood at all or even the past year, its like my memory resets every few months, and i dont remember major things either, then like everyone in this generation, I can focus or study for more than half an hour and even then i forget it after exams (which i somehow pass).

I had gotten into my first relationship last year and things have went downhill after that, my depression felt like it was cured but my anxiety multiplied ten fold, I was worried about him and his love and if he’ll leave - que abandonment issues, possessiveness, and telling him he doesn’t care or love me over the smallest things like being delivered for hours. Ultimately he got sick off it, and I’ve been feeling like dying and even though i know he doesnt want me I’ve been going back just to get hurt (i think he’s mentally unstable too tbh donno if its bc of me) but its like i cqnt control myself, even if i dont want to, I keep begging and acting crazy, the more he pushes me the more i have to pull. I’m showing signs of BPD or so I think? I want to get diagnosed so bad, just so i know its not my fault or that its my mental state you know?

I’m in a small place in India and it all ties together to me not getting the help I want/need. What do I do? I feel helpless.

Work rants?

Warning: self harm

I think everyone here is working, and here's my short rant.

Looking at the CCTV because of my lost wallet, last seen was when it was on me.(Place I work, I'm a salesman) Then, Manager suddenly in all caps locks and asks if I just hurt myself behind my senior backs. It was before all of this wallet situations.

My senior says what if we work 9 Am till 11 pm, one day off per week, I say that would be illegal. He says 'Are you sure? Even I get paid?"

I said yes, he then pushes me by saying where I learn it from, which law part, sector and what conduct or code it is.

my senior says that I don't have any educations, no formal educations and compare me to the cashier with like 6 SPM A's, 17 and by the end of the month going to college and says even if she's quite annoying sometimes and shout a lot, his remark is 'you can tell she have educations' and ask if I'm nothing. In my opinion truthfully, said. "Yes"

I became slightly unstable, so after some time, I saw the cutter box and tested the sharpness by cutting myself slightly, nobody saw it, until my senior saw I suddenly had new scars(it was extremely small compared to my scars). Right before he confronts, his friends call, it ends.

This is where the wallet part is gone.

Anyways, back to right now, my manager asks why, in which I reply it's my condition, which is why I have prescribed medications(clonazepam) reply again, manager threaten me to not do it again, and says like he doesn't care about this and if he this 1 more time, I will get terminated.

After that, I reply, "Understood" and take a picture, and just delete the whole chat before that whole thing happens.

Now I feel nothing yet my body is still slightly tense, I'll just prepare myself when I finally go back home to report the police about my lost wallet/IC and call my banks about my debit card.

Any Adult Neurodivergent/ Autism community groups around KL?

Late 30s M here. Exploring whether I might be neurodivergent

I only heard the terms Neurotypical/Neurodivergent fairly recently, after a therapist brought it up during a counselling session and, I think, was gently hinting that I might be neurodivergent. That’s when I started learning that it can include things like Autism and ADHD.

Even before that, over the past few years, I’ve occasionally wondered whether I might be on the autism spectrum, particularly because I’ve experienced social awkwardness and anxiety throughout much of my life. But I never really looked into it deeply—perhaps partly because I was avoiding the possibility of finding out.

Now I’m slowly trying to face it and understand more about neurodivergence and Autism. The more I read, the more I find myself relating to certain things when I reflect on my past. Obviously I don’t know whether I’m actually autistic, and I’d like to see a professional for a proper assessment eventually.
Unfortunately, I’m not in a good financial position right now to afford an assessment.

So I’m wondering: are there any adult neurodivergent/autism support groups in Malaysia, either online or in person, where I could learn more and talk to people with similar experiences?
Would appreciate any recommendations or experiences.

Please help, genuine, I don't know what to do with life anymore. I'm so lost. There's no one to talk to.

I don't know what to do. I feel so lost. I don't know where to begin. But since I'm typing, maybe I can make it work somehow.

I'm a student, and I struggle a lot with deadlines. Sleepless nights, drawing day to night, lots and lots of assignments.

Recently, there was this girl that I like in my class. I was very very obvious with my actions that thw whole class realize it. At first she didn't realize, but eventually she does. She's nice to me and all, and despite knowing how I feel, she's very respectful about it. We occasionally talk and everything, but recently things got awkward, because classmates started talking about it. I realized her awkwardness, and I thought I was being annoying. I felt so down at that time, I was silent for a few days. Few days later, there's another classmate talking to me in secret about how other classmates (including the girl I like) might be secretly dismissing her aside. (Note : I know all about these secret, the girl that I like told me about being careful not to get manipulated by this classmate.) I listened to her problem and there was NOTHING i can do about it. I can only lie to her by saying "idk, i didn't realize". If I do reveal, the entire situation will turn into chaos. All I can do is to keep this secret and this guilt in me along, faking a two sided face. At this rate, I have 3 assignments to rush in a week. All these assignments are not something can be done in a week. In this entire week, I was silent. In my brain, all I am thinking is "there is no reason to react. Even if i react, nothing will change. All I can do is to do what I'm supposed to do". Telling them it's wrong to just dismiss someone like that : that classmate did something wrong, I can't deny that, I can't just tell them to hold it in. Telling that classmate she's treating them badly : she realize, confronts them, everything goes chaotic. No reason to do neither, it'll cause a scene. Being nervous to assignment : there's no reason to. Being nervous will not help with the situation. Crying : there's no reason to. Crying doesn't help with the situation, it'll be in my way of doing assignment. Laughing : there's nothing to laugh about, focus on your assignments. Not knowing the consequence, I think I accidentally flipped a switch in me. It killed off all emotions. I had a very bad day today. Presentation went extremely bad. Partly because I feel lost. Partly because the work I did didn't really have much thought put into it. I thought "I wanna go back home and cry to sleep, wake up and finish off this other 2 assignments." But i can't. I physically can't cry. I don't feel anything. No fear, no nervous, no happiness, no sadness.

The whole story up there is just to vent, and to add context. I just want to reactivate my emotion again. Please help me.

Need help for Adhd diagnosis in public clinic / hospital

I've noticed major Adhd symptoms throughout my life, and I just want to diagnose it. Since public hospitals are the only option, I have a few questions

-How long does each session take

-How can I fully maximise my sessions

-How long until I can get diagnosis results

If you have any tips or experiences please share with me thanks

How do i get help for my mental health in Perak?

I've been struggling with suicidal ideation, have felt alienated and been bullied throughout my life. I'm currently studying for my Diploma and I'm trying my hardest to do well but I cannot take it anymore because of how constantly depressed Ive been over the years. I've done research and I suspect myself with ADHD and MDD, my suspicion with neurodivergency is possible because my brother has Level 1 ASD. I also would rather not tell my family a single thing about my mental health issues, which is why i'm doing this alone.

Currently I have two plans:

  1. I was planning to take the test that people recommend on MySej and go to a KK, but I've heard a lot of bad experiences with going to a KK. I want to get assessed in a private clinic, but I also do not have money to pay hundreds per checkup. Is there any private clinic specifically in Perak that is affordable and kind to their patients? I specifically would prefer Batu Gajah or Ipoh, but anywhere in Perak is fine too.

  2. Once I enter a University in KL for my degree, only then will my family be able to provide a lot of money for me. If I dont end up getting any mental health help during my diploma, then i plan to go to a well rated private clinic.

I know the second option sounds the most logical, but I honestly cant bear it anymore. I've been stuck in this cycle of either numbness or extreme emotional sensitivity, I always end up engaging in self-harm and it's hard to stop. I'm desperate and I want to find any private clinic that can assess me along with a diagnosis and refer me to a psychologist with an affordable price if possible.

Thinking about asking a doctor about propranolol for public speaking, is this a good idea?

I’m 19 and currently involved in student organizations, so I have to attend meetings [like a ton], give presentations, and speak in front of people fairly often.

The main issue is that when I have to speak publicly, my heart starts beating really fast, my hands can shake, and sometimes my voice trembles. I can get pretty nervous even when I know what I’m supposed to say.

I’ve been reading about propranolol because I heard it can help with some of the physical symptoms of performance anxiety. I’ve never taken it before and I’m thinking about having an online consultation to ask whether it would be suitable for me (DoctorOnCall).

I don’t have any known medical conditions or allergies, and as far as I know my blood pressure and heart rate are normal. I currently take fish oil, shilajit and creatine.

For people who have dealt with something similar, do you think it’s reasonable to bring this up with a doctor and ask about propranolol? How did your consultation go

My abuser wants to meet my psychiatrist, what do I do?

(I am an adult but financially reliant on my abuser, as Im currently studying in university and would like to at least have higher education done before I plan my escape.)

My abuser looks through my stuff, and is the reason why I have a PTSD diagnosis. My psychiatrist is from a government hospital (and with gov hospitals, it’s never the same doctor so it’s quite a gamble)

My abuser wants to force me to make an appointment with my psych so they can talk with my dr about my behaviour (mostly because they think I’m being ā€œlied toā€, that my doctors are actually ā€œenablingā€ my behaviour) . I know I can just decline and refuse to do so as I am an adult, but I’m afraid of the consequences of not complying (getting pulled out of university, getting kicked out of the house etc). I don’t want them to learn about my PTSD diagnosis because it will worsen my abuse. I don’t want them to discuss with my doctor because I know some psychiatrists are biased and will side with the family.
