The glass exploded

Had to watch it a few times to see what he hit.

I worked with glass for 5 years in a manufacturing plant.  There was a raw edge that didn't get "seamed" correctly, when part of that edge touched the metal edge of the shelving that's what started the microscopic chip, that turned to a break from the mass of the glass.  

That's my guess anyway.

It's a dumb rack. High up, metal framework, no pads (that I can see). Wooden a-frames are the industry standard for a reason.

The guy isn't even wearing safety gear. Shop is just one huge lawsuit waiting to happen.

Industry standard...ehhhhh.  Not in the 2 glass factories I've worked at.  1 made automotive glass for all the big brands (Ford, Chevy, Dodge, Mercedes , etc).  You'd be surprised how glass gets packed and shipped. 

But also, yes you're correct that their setup is absolutely horrible and dangerous.
